If you wish to register with us, please telephone the surgery. We will need a completed Registration form which you can arrange to collect from us or you can download a form via the link below.

We also ask for your National Health Number which is a unique ten digit number allocated to your health records. You can find this on any letter or document from the NHS, including prescriptions, test results or your NHS App. If you cannot find your NHS number at home, you can ask your current GP practice to help you.

One of our Reception Staff here will check your completed form and also that you are within the Practice boundary.

If you are on any medication you will need to discuss this with your new registered GP.

After completing the registration forms we will inform the Health Authority of your new details and your notes will be retrieved from your previous GP.
